"Fore the Vets" Hole-In-One Prize

Falvey Insurance Group is thrilled to have our Hole-In-One prize for the "Fore the Vets" charity golf tournament sponsored by Viti Automotive. Anyone who makes a hole-in-one on the 13th hole at Newport National during the tournament, and meets the below terms and conditions, qualifies for a:

Terms & Conditions

Important execution requirements per the terms & conditions include, but are not limited to:

  1. YARDAGE - Signage provided for your tournament by AHNO states the prize and displays a hole number and minimum yardage. The main prize hole(s) must play from the minimum yardage listed on the contract and printed on the prize sign(s), regardless of tee box locations. IMPORTANT: It may be required to move the tee boxes to achieve the correct yardage.
  2. WOMEN GOLFERS - On main prize hole(s), women may play from a maximum of 15 yards closer to the hole than men up to a minimum distance of 135 yards, regardless of tee box locations. IMPORTANT: Women's tee boxes may be required to be moved to the correct distance prior to the start of the tournament. (On bonus prize hole(s), women may play from the ladies’ tees)
  3. HOLE # - The signs provided by AHNO must be displayed on the correct hole number as stated on the contract. Tournament officials are responsible for confirming the prizes are displayed on the correct hole numbers. There is no valid prize coverage for a main prize placed/displayed on an incorrect hole number.
  4. WITNESS REQUIREMENTS – For prize limits up to $5,000, one Official Witness is recommended but not required. At a minimum, the shot must be witnessed by at least two Playing Partners. Please note the testimony of independent, non-playing witness(es) is always helpful in the event of a claim. 2) For prize limits $5,001 to $20,000, a minimum of one Official Witness is required. The witness should be in a location that allows him/her to see golfers on the tee and the actual hole in the ground on the green. Their dual role will be to verify the claimant only took one shot from the correct tee marker / distance and see the golf ball go into the hole. 3) For prize limits $20,001 to $100,000, two Official Witnesses are required. One should be positioned at the tee-off area and the other adjacent to the green, out of harm’s way. Having witnesses in these locations is the ideal scenario. 4) For prize limits of $100,001 or more, two Official Witnesses are required, positioned in the same manner as described above, one of whom must be a PGA-certified professional or law enforcement official.

IMPORTANT: If there is a failure to abide by minimum Official Witness requirements, the claim is subject for denial.

  1. NO MULLIGANS - No mulligans, practice, or additional shots of any kind are allowed at any time on a main prize hole. Each golfer may have a maximum of one attempt toward a hole in one prize regardless of the outcome of the shot. A golfer who hits a second shot due to lost ball, hazard, etc., or is 'rounding out' a foursome by taking an extra shot for the team is only eligible on his or her first shot. (Mulligans may be taken on bonus prize holes, but prizes are only awarded for their first shot)
  2. AMATEUR GOLFERS ONLY - Hole in one prize coverage is valid for amateur golfers only (according to USGA guidelines), unless declared prior to tournament and stated on contract issued by the insurance company.
  3. NUMBER OF GOLFERS – the insurance company and sponsor must be notified prior to the tournament if the number of golfers varies by more than 5% from the number of players listed on the contract. (A maximum of 5% variance is permitted without contacting AHNO)
  4. TOURNAMENT PLAY ONLY - Prizes and signs are for tournament/event play only. Signs should be placed at the holes immediately prior to the event and should be removed after the final player has played a par 3 hole. No claims will be considered for players who are not officially registered with the contracted tournament or event.
  5. EVENT CANCELLATION - If the event is canceled or rescheduled, you must contact AHNO as soon as possible and no later than the first business day following the originally scheduled event date.
  6. PRIZE CLAIM FOR A HOLE IN ONE - If any eligible golfer successfully makes a hole in one on any prize hole, immediate telephone notification must be made to the sponsor so we may report this to the insurance company no later than the first business day following the event. A signed and notarized affidavit, recorded telephone interview, and/or polygraph test by a professional polygraphist may be required from witnesses and/or tournament administrators to verify the facts of this claim.
